 Appointments
Please call the office at (317) 770-1700 between 8:30am and 4:30pm, Monday through Friday, to schedule an appointment. If you are unable to keep your appointment, please call us as far in advance as possible in order for us to reschedule that time slot. Missed appointments will be charged a no show fee. We will make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgeries.

As directed by our office, please download the form(s) and bring the completed form(s) to your scheduled office visit. This will help expedite the registration process. Thank you.

 Fees and Payments
It is customary to pay for charges at the time of service. Cash, checks and credit cards are accepted. Our office will bill insurance companies directly and assist in handling patient claims when requested by patients. We recommend that patients verify their coverage with their insurance carriers prior to receiving medical care. Remember that benefits are a contract between the patient and his/her insurance provider. Payment for services is the patient's responsibility and he/she will be responsible for any balances not paid by insurers.
Insurance
We are participants in most area insurance plans including Workmen's Compensation. Our office staff will gladly assist patients in identifying coverage. It is the patient's responsibility, however, to know and follow their insurance plan and requirements.
Prescriptions and Renewals
Please request all prescriptions and authorizations for renewals during normal office hours when our full records are available.
Presentations
Group talks and slide presentations are available by contacting the office. Subjects include joint replacement, arthritis, sports medicine and orthopaedic injuries in the work place. Free literature is available in the office.
